Monza headstock sticker

I'm restoring a 160 monza jr. that's missing the frame VIN sticker. Can anyone post a picture of a completed headstock id sticker so I can create a reasonably accurate reproduction?

yes, but what do I type in the blanks to make it look correct to the DMV?

the numbers on your VIN

engine and frame the same

the other info you can get from the owners manual

I'm looking at one off a 250 so I'd say replace the 250 with 160... but here's what the numbers are

250 (type?)
196 (this is year, the last digit was left blank by Ducati to be filled in by the dealer when sold. Fill it in if you like)
Engine Number
Frame Number (they match)
125 (weight)
150 (max load?)
250 (cylinder displacement)
